Accessing your own recordAs part of our commitment to providing quality care, you have been given the opportunity to access your electronic health record. In turn, you may allow those close to you to also access your information. If you decide to get online access you can: |

Access for parents and guardiansIf your child is participating in Healthelink you can get online access to their record, by becoming an associate, if you are their parent or a legally authorised guardian. If you decide to get online access you can:

Giving access to other peopleYou may want to give other people access to your record so that they can see your information or even add to it. For example your partner, parents, children or other significant person. We call these people ‘associates’ and with your consent they are able to view your record online. You can give someone else access to your record by completing the pink Request for Associate Access to Record Form |
