IMPORTANT NOTE: Not all of your child's health care providers will necessarily look at their record. So if you have added something to your child's Healthelink record that you want their health care provider to be aware of, you must tell them at their next visit.
Only a selected number of post offices can process the Request for Parental Access to Child's Record Form. These are the majority of Post Offices in and around the pilot postcodes. A list of these post offices can be seen by clicking on the link below:

To ensure that we give access to the right person, we need proof of your identity, your child's identity and also proof of your relationship with your child.
To prove your identity, you must present at least two original documents from the table below that adds up to 100 points.
Only one identity document worth 70 points can be used.

An Australian full birth certificate showing parental details, or a current photo birth card issued by the NSW Registry of Births Deaths & Marriages Or an overseas birth certificate showing parental details, provided a passport or an official Australian travel document is also shown |
