How to view your Electronic Health Record

How do I get online access to my Healthelink record?
If you would like to get access to your Healthelink record, you need to:
- Complete the Request for Access form enclosed in the information kit that you received when you were enrolled. If you have not recieved an information kit or the Request for Access form contact us,
- Take the completed form to a participating post office.
- You must attend the Post Office with your proof of identification. (See below for details of the documentation required).
- After the form is submitted you will be contacted by us after approximately two weeks, at which stage you may call the Healthelink Service Centre to be provided your login and password details.
Giving others access to your Healthelink record
If you’re taking part in Healthelink you can choose to give another person access to your record. You might want to give access to your husband or wife, carer, son or daughter, or someone else you think needs to see your health information.
These are two different groups of people that can be associates to a Healthelink record: those who have legal status as authorised representatives and those who the participant consents to having access.
Authorised representatives
Authorised representatives are people who have the legal authority to act on behalf of someone else – such as partner, son, daughter or carer who is authorised to act on behalf of the patient.
If you are applying as an authorised representative Healthelink needs to see proof of your identity.
If you are applying as an authorised representative of someone participating in Healthelink that person does not need to attend the registration point with you, but you do need to provide proof of relationship to the participant.
Associate persons
Healthelink allows you to give any person access to your record by making them an ‘Associate’ person.
If you want to Associate another person to your record they must attend the Healthelink registration point with you and be registered into the system. You need to choose carefully who you nominate as an Associate person – once nominated, the associate will be able to view and add information to your Healthelink record.
Associates will have their own Healthelink record, which they can add information to and which can be viewed by participating healthcare providers. However, unless the Associate also meets the criteria of the pilot no clinical information will be automatically added to their Healthelink record.
Please note: To ensure the privacy of Healthelink records you will only be granted access to your own record and the records of anyone you have legal responsibility for or who has consented to your access.
What information will an Associate person be able to see in my record?
An associate person can see your entire Healthelink record. They will have the same level of access to your record as you and can view and add information to your record in the same way you can.
Can I give access to more than one associate person?
Yes. You need to follow the same registration process for each person you wish to associate to your record.
Are there any restrictions on who I can give access to?
No. You can give access to anyone you think needs to see your Healthelink record. However, you should carefully consider the decision as the person granted access will be able to see everything on your record.
Can I have access to the Healthelink record of someone I care for?
Yes, if you can show that you are an authorised representative of that person or if they provide consent. If you are unsure whether you are an authorised representative, contact Healthelink for advice.
Can an associate person delete information in my record?
No. No-one can delete information in your record. Associate persons can add notes to your record, just as you are able to add information to your record.
What if I want to remove an associate person’s access to my Healthelink Electronic Health Record?
To remove an associate person’s access you need to contact Healthelink and request that the association is removed from your record.
How do I give someone access to my record?
If you are nominating someone for associate access, you need to:
- Ask the Associate to complete the Associate Request for Access form.
- Take the completed form to a participating Post Office. Both you and your Associate are required to attend the Post Office.
- You must attend the Post Office with your proof of identification. (See below for details of the documentation required).
- After the form is submitted your Associate will be contacted by mail, within approximately two weeks, after which you may call the Healthelink Service Centre to be provided your login and password details.
Identification documents
Primary list (extracts are not acceptable):
At least one of your identification documents must be from this list and must show your date of birth:
- An Australian full birth certificate showing parental details, or a current photo birth card issued by the NSW Registry of Births, Deaths and Marriages.
- An overseas birth certificate showing parental details provided a passport or an official Australian travel document is also shown.
- A current Document of Identity issued by the Australian Passport Office.
- A current Australian passport or one that expired within the last two years.
- A current overseas passport.
- An Australian naturalisation or citizenship document or immigration papers issued by the Commonwealth Department of Immigration and Multicultural and Indigenous Affairs.
- An RTA-issued NSW photo driver licence that expired no more than two years ago.
- A current photo driver’s licence from another Australian state or Territory or one that expired within the last two years.
- A current photo identity card for NSW police force officer or Australian defence force, excluding civilian staff or family.
- A current consular photo identity card issued by the Department of Foreign Affairs and Trade.
- A current NSW Photo Card or one that expired within the last two years.
Secondary list:
- A current Medicare card, Pensioner Concession Card, Department of Veterans' Affairs entitlement card or any other current entitlement card issued by the Commonwealth Government.
- A current credit card that shows your name, or account card from a bank, building society or credit union, or a passbook or account statement up to one year old.
- A telephone, gas or electricity bill up to one year old.
- A water rates, council rates or land valuation notice up to two years old.
- An electoral enrolment card or other evidence of enrolment not more than two years old.
- An armed services discharge document up to two years old.
- A current student identity card or a certificate or statement of enrolment up to two years old from an educational institution.
- A current plastic 'licence-style' Mobility Parking Scheme (MPS) card, with or without a photo.
Name changes:
If your name is not the same on both of your identification documents then you need to provide documentation which shows a clear link between your names. Documents Healthelink will accept as proof of your name change are:
- A marriage certificate (commemorative certificates are not acceptable).
- A divorce decree (bearing the name being reverted to).
- A deed poll registered with the relevant authority.
- A change of name certificate issued by the Registry of Births, Deaths and Marriages.
- A birth certificate showing the name at birth and the new name (commemorative certificates are not acceptable).
- A document showing evidence of change of name registered in the Land Titles Office.
Proof of relationship:
If you are applying as an authorised representative, you will need to provide both identification documents for that person andf proof of the relationship. This can be:
- A current legal document giving authorisation to act on behalf of the person (both names must appear on the document). This may be a power of attorney.
