How to view your child's Electronic Health Record

The information in Healthelink exists primarily for participating healthcare providers that are caring for your child. In addition, you may also apply to get access as an "Associate" to view and manage their record.

Getting access to your child's Healthelink record

If your child is participating in Healthelink you can choose to get online access to their record if:

  • you are their parent; or,
  • you are a legally authorised guardian.

If you choose to get online access you may:

  • review a summary of your child's health information (it is the exact same view that healthcare providers have),
  • add additional information,
  • view an audit of who has looked at their record.

While the project is being piloted not all healthcare providers your child might see will necessarily look at their record. Important point: If you have added something to your child's Electronic Health Record that you want their healthcare provider to be aware of, you must tell them at the appropriate time.

  

How do I get access to my child's record?

If you would like to get access to your child's record, you need to:

  1. Complete the Request for Associate Access form enclosed in the information kit that you received when your child was enrolled. If you have not recieved an information kit or the Request for Access form contact us,
  2. Take the completed form to a participating post office,
  3. You must attend the Post Office with both your proof of identification as well as that for the child and you must provide proof you are their parent or legal guardian. (See below for details of the documentation required). You don't have to take the child to the Post Office,
  4. After the form is submitted you will be contacted by us after approximately two weeks, at which stage you may call the Healthelink Service Centre to be provided your login and password details.

My child getting access to their own Healthelink record

When your child turns 16 they will be able to manage their own Healthelink record if they choose, including getting online access. The process is identical to above.

 

We will write to them just before their 16th birthday to advise them of their options.



Associate persons

Associates will have their own Healthelink record, which they can add information to and which can be viewed by participating healthcare providers. However, unless the associate also meets the criteria of the pilot no clinical information will be automatically added to their Healthelink record.

 

Please note: To ensure the privacy of Healthelink records you will only be granted access to your own record and the records of anyone you have legal responsibility for or who has consented to your access.

 

These are two different groups of people that can be associates to a Healthelink record: those who have legal status as authorised representatives and those who the participant consents to having access.  These are described below:



Consenting Access

There are instances where the participant can give consent to allow someone other than their parent or guardian access to their record.

 

When a child turns 16, Healthelink will write them a letter inviting them to get their own User ID and Password to access their record. They will then be able to make decisions regarding who has access to their record.

 


What information will I be able to see in my child's record?

Associate persons have the same level of access to your child's record as healthcare providers. You can view and add information to their record in the same way.  

 
Can more than one person be associated to my child's record?

Yes. You need to follow the same registration process for each Associate person that wishes to have access.




Are there any restrictions on who I can give access to my child's record?

No. Once you are an Associate you can choose to give access to anyone you think needs to see your child's Healthelink record. However, you should carefully consider the decision as the person granted access will be able to see everything on their record.

  

Can I have access to the Healthelink record of someone I care for?

Yes, if you can show that you are an authorised representative of that person. If you are unsure whether you are an authorised representative, contact Healthelink on 1800 75 22 66 for advice.

 

Does an Associate person become part of the Healthelink Electronic Health Record pilot?

Yes. Once the Associate person has been registered Healthelink will give them a unique username and password that will allow them to access their record and the record of the child. This username means the Associate person will also have their own Healthelink Electronic Health Record that they can add personal health information to and which can be seen by participating healthcare providers. However, unless the Associate person is also eligible to participate in the Healthelink Electronic Health Record pilot (ie. within the pilot age and postcode groups), no information from consultations with healthcare providers will be added to their record.


Can an associate person delete information in my child's record?

No. No-one can delete information in your child's record. Associate persons can only add notes to records.

 


What if I want to remove an associate person's access to my child's Healthelink record?

To remove an Associate Person that you chose to be an Associate you need to contact Healthelink on 1800 75 22 66 and request that the association is removed from your child's record.

 

 
 Identity Documents:

  

Associate to provide 100 points or more worth of Identity Documents.

 

Documents worth 70 points each:

  • A birth certificate.
  • A current passport or one that expired within the last two years.
  • An Australian citizenship certificate.

Documents worth 40 points each:

  • An RTA-issued photo driver licence that expired no more than two years ago.

 Documents worth 25 points each:

  • A current medicare card.
  • A current credit card that shows your name, or account card from a bank, building society or credit union up to one year old.
  • A telephone, gas, electricity bill up to one year old.
  • A water rates, council rates or land valuation notice up to two years old.
  • Public service employee ID card.
  • Mortgage documents.
  • A current photo card or one that expired within the last two years.
  • Bank passbook or statement (only one documents per financial institution).
  • Centrelink card.
  • Tertairy education student ID card.
  • Property rates notice.
  • Marriage certificate.

 

Identification of your child:

  • A current passport or one that expired within the last two years.
  • A birth certificate.
  • An Australian citizenship certificate.

 

Name changes:

If your name is not the same on both of your identification documents then you need to provide documentation which shows a clear link between your names. Documents Healthelink will accept as proof of your name change are:

  • A marriage certificate (commemorative certificates are not acceptable).
  • A divorce decree (bearing the name being reverted to).
  • A deed poll registered with the relevant authority.
  • A change of name certificate issued by the Registry of Births, Deaths and Marriages.
  • A birth certificate showing the name at birth and the new name (commemorative certificates are not acceptable).
  • A document showing evidence of change of name registered in the Land Titles Office.

 

Proof of relationship: 

If you are applying as an Associate, you will need to provide identification documents whioch prove your relationship. This can be:

  • The child's birth certificate or equivalent (such as adoption papers) showing both the child's name and your name.
  • A current legal document such as a court order or equivalent giving you legal authorisation to act on behalf of the person (both names must appear on the document). This may be an enduring power of attorney or a parenting order or some other legal document.
